Academy of Music Registration
Please complete all three:REGISTRATION form, STUDENT AGREEMENT form, and PAYMENT form. We recommend paying on-line or by pre-authorized payments.

By Mail: Complete ALL THREE forms and mail with PAYMENT to: Academy of Music, West Highland Church, 1605 Garth St., Hamilton ON L9B 1X8
In Person: Complete ALL THREE forms and place with payment in an envelope. This can be given to our Welcome Centre volunteer on Sunday mornings or you can pay in person at the church office Monday to Friday between 9:00 A.M. to 5:00 P.M.
Pre-Authorized: Pre-authorized payment (monthly or by term) is encouraged. Pre-authorized forms can be found at the West Highland foyer, office, or can be downloaded. Please follow the instructions for submission.
