By Mail: Complete BOTH REGISTRATION and STUDENT AGREEMENT forms and mail with PAYMENT to: Academy of Music, West Highland Church, 1605 Garth St., Hamilton ON L9B 1X8
In Person: Complete BOTH REGISTRATION and STUDENT AGREEMENT forms and place with payment in an envelope and hand it to the attendant at the Welcome Centre at West Highland Church. Alternately you can pay in person at the West Highland Church Office (1605 Garth Street) Monday to Friday during business hours 9:00 A.M. to 5:00 P.M.
Pre-Authorized: Pre-authorized payment (monthly or by term) is encouraged. Pre-authorized forms can be found at the West Highland foyer, office, or can be downloaded here. Please follow the instructions for submission.
